In-Network: This is when we have a contract with your insurance company and conform to all requirements of your insurance company. We bill your insurance company directly and submit all necessary information. You are responsible for paying the deductible, coinsurance and/or copayments. This is often the cheapest option if you have a low deductible, coinsurance or copayment.

Out-of-Network Providers

While we'd like to be in-network with as many insurance companies as possible, we are only able to accept insurance plans that allow us to maintain a sustainable and equitable practice. Even if we do not have a contract with your insurance plan, you may still have out-of-network benefits - In this case, your insurance may still pay a portion of your bill.

Out-of-Network: This is when we do not have a contract with your insurance company or a particular plan. In this case, we first call your insurer to verify your out-of-network benefits. Second, we explain exactly how much our services cost and how much you will pay after your insurance pays its portion. This will often cost you more than if we were In-Network.

Self-Payment Rates

Please ask about our self-payment rates for:

  • Physical Therapy

  • Dry Needling

 

Payment is due at the time of treatment using cash, check, credit card, HSA, FSA.

Self-payment: This is when we provide one-on-one, unrestricted physical therapy in exchange for up-front payment.  At your request, we will supply you with a superbill that you may submit to your insurance company for repayment. In this arrangement, the insurance company may or may not agree to reimburse you. (See the reasons some choose the Self-Payment option)
